Awana is a Bible club for children and will begin September 30 at Northern Lakes Church in Cumberland, from 5:30-7:00 pm each Wednesday.  Awana is for children 3 years old through 4th grade.  There will be games, music, Bible lessons, and Bible memorization.  Dinner will be served from 4:30 to 5:15 pm for a free will offering, beginning September 30. 

Thank you for entrusting your child/children to us.  At Northern Lakes Church, we are committed to providing a safe environment where children can grow in their love and knowledge of God.  Even in places that are designed to keep children safe, children can find ways to get hurt.  By submitting the following form, you are agreeing to the the following release of liability before leaving your child/children with us.  You are also giving your permission for us to use your child/children’s photo as described below.  Thank you, for bringing your child/children to church; we look forward to working with them!     

1.     Photo release. I give permission to Northern Lakes Church to use my child/children’s photo for classroom projects, for decorations in the children’s wing, for helping with attendance and/or church website.                                                                    

2.     Release of liability.  I, for myself, my minor child and for the child’s other parent and/or guardian, hereby release, waive, discharge, and covenant not to sue Northern Lakes Church, and its officers, director, employees, agents, volunteers, heirs and assigns of and from all liability, loss, claims, demands, possible causes of action, court costs, attorneys’ fees and other expenses arising from any lawsuit that may otherwise occur from any loss, damage or injury to my child’s person or property in any way resulting from or connected with my child’s attendance in the Children’s Program at Northern Lakes Church, including, without limitation, the failure of anyone to enforce rules and regulations, failure to make inspections, or the negligence of other persons.  

3.     Consent to Medical Treatment. In the event my child becomes ill or injured, I give consent for a responsible person at Northern Lakes Church to administer first aid to my child/children as needed.  I also give consent to secure additional medical treatment for my child/children under the following conditions: reasonable effort has been made to find me at church or to call me with the provided phone number; or NLC staff determines that care is needed prior to the probable return of the parent/guardian to pick up the child.
